Add an Athlete to Your Existing Account

If you currently have a Healthy Roster account and need to add an additional athlete to your preexisting account please complete this form.

• This form must be completed for each additional child that you wish to link to your account. Be sure to list the email address that you already use to login to your existing Healthy Roster account, that way your athletes’ profiles are linked correctly.
• Please only submit this form once. Due to limitations on the Athletic Trainers office hours, it could take up to a week to have your new athlete(s) added to your account.

Once your athletes are added to your account, you will see their profiles the next time you login to the website or app. You will not receive an invitation code via email like you did when you first created your account.
